Registration Information
Registration for new students occurs each year during Catholic Schools week in January. Students will continue to be accepted into classes as space dictates. When necessary, a waiting list will be established for students who hope to gain admission. At the time of registration, the registration fee and registration deposit are required.

Registration Instructions For New Students
1.
Complete the registration form. Do not leave any spaces blank.
2.
A non-refundable $200 registration fee for each child is required at this time. The fee is not applied to the tuition cost. A $200 deposit is also due at the time of registration. This deposit is also non-rufundable. The deposit will be deducted from the tuition balance.
3.
Complete the medical information sheet.
4.
Attach a birth certificate (Photo copy is acceptable.)
5.
Attach baptismal certificate. (Photo copy is acceptable.)
6.
All students are required to have an updated physical examination. Physical exams must be completed and received by the school office prior to the student’s entrance in September.
7.
Kindergarten students (five-year-olds) must participate in a kindergarten screening process. We will notify you of the date and time of your child’s screening.
8.
Any student entering grades 1 – 8 must furnish the office with a copy of the previous year’s complete report card and a copy of the current report card showing the first two quarters. If a student has had a CORE evaluation, you must also provide a copy of the current IEP.
9.
Complete the Release of Records Authorization form for any student in grades 1 – 8 and forward it to your child’s present school. St. Mary’s must receive the records before your child can begin.
10.
An additional deposit of $200 per student is due on May 1. This will also be deducted from the tuition balance.
11.
Pre-school requires a separate deposit.
